Dental Work Appointments

Insurance Payments: As a courtesy to our patients we provide processing of your insurance
claims. To do this, however, we will need your dental insurance identification card and a
breakdown of benefits, which can be found in your insurance company's manual.

Other Forms Of Payment: We also accept cash, personal checks, Visa, MasterCard, Discover,
Dental Fee Plan and CareCredit. Dental Care and CareCredit are revolving charges which allow
for flexible, interest-free/low interest term payments. Both plans offer a quick and easy application
process, which can be done either online or in our office. In most cases, qualified applicants will
be approved within ten minutes!

Payment for services is expected at the time of each visit.
